Chronic Conditions Management
Tackling demand within the NHS

The CCM strategy set out in improving Health and the Management of Chronic Conditions in Wales: An Integrated Model and Framework for Action (WAG, 2007) recognises that services are currently unsustainable with an over reliance on traditional, and often inappropriate, models of care. Advance learning and evidence gathering to inform implementation and mainstreaming of the strategy is carried out in three CCM Demonstrator Projects in Carmarthenshire, north Wales (Gwynedd) and Cardiff.
This paper has been produced to share experiences of the use of geographical mapping tools to support the development of locality working. Gwynedd is a geographically diverse area and planning for equitable access to services is a complex task. A range of maps were produced to describe the three identified localities, showing key relationships and helping to inform service redesign.
