Efficient Chronic Conditions Management

One of the largest Health Boards in Wales, Abertawe Bro Morgannwg University Health Board have invested in Total Solution Mapping (TSM). 

Serving a population of 600,000 in the areas of Bridgend, Neath Port Talbot and Swansea; Abertawe Bro Morgannwg University Health Board will be using the system to manage their patient flows and to look at travel times around their estates.

The system will be embedded in their Chronic Conditions Management Team and will act as a powerful tool for them to add efficiencies and cost savings to their service. By using the latest and most relevant information, the Health Board will ensure that they can meet varying and ever changing patient needs.
